Text: | 05/11/17 having difficulty getting iCart to accept payment, will try again or mail check. Entrance will be to lake lot, would like to discuss usage prior to construction. ckr 05/12/17 payment has been received. to maintenance for review. ckr 5-15-17 There is already an approach on the property. I spoke to Mike on the phone. I informed him we would not allow the additional approach. He asked what other options he has. I said he could remove the additional approach and remove the main approach and move it west on top of the hill to improve sight distance to the east and west. He asked me to send him a copy of the policy. JD 5-23-17 Mike called and asked if I could look at the approach. 5-24-17 I looked at the approach. he wants to move the existing east approach to the west , just west of the power pole. I called Mike and left a message that we would like to see it further west yet but if he can go as far west as he can to improve west sight distance that would be great. I also said he cannot use the plastic pipe in the ditch now. He can reuse the pipe under the existing approach when he moves it. He must completely remove the existing approach, place topsoil, seed and when it has established vegetation I will look at it to determine if his deposit can be refunded. I also said he has to cut down the back of the ditch slope so the approach comes out flat and our plows do not hit it. JD 7-26-17, looked at the approach. The old approach has no topsoil and is severely washout out. The new approach has some seed established on the west side and nothing on the east side. I spoke to Mike and informed him of this. I said he needs to remedy the washout immediately or we would have to and assess him for the repairs. He said he wanted the deposit back so he could fix the other part. I said I would not do that and he needed to fix the washout immediately. JD 6-12-18 Inspected and ok to release the deposit. All looks satisfactory. JD 06.28.18 Request to Finance to issue $475.00 refund. ckr |
